Pack your bags and your favorite sunnies—it’s time to book your trip! But first, here’s what you need to know: Submitting information via this reservation form does not automatically confirm your reservation. We will put your trip on hold then email you an invoice for review & approval within 1-2 business days.
It is your responsibility to review the invoice upon receipt to ensure 100% accuracy, so please check thoroughly for any discrepancies or errors. DWD Travel is NOT responsible for errors, or any fees associated with necessary changes after 24 hours of receipt.
* IMPORTANT! Please enter your legal name(s) EXACTLY as appears on your valid passport. Additional fees may apply if a reservation has to be changed due to incorrect information. DWD Travel is not liable if the information below is not entered correctly.
ARE YOU USING A DEBIT CARD?
Most debit cards have daily charge limits. Depending on your bank, this limit may be as low as $500. Please confirm the daily charge limit with your bank prior to submitting a large debit card payment. If you attempt to charge over your daily limit, your card will be declined—and that’s no fun for anyone.
DOES YOUR PASSPORT EXPIRE WITHIN SIX MONTHS OF YOUR TRAVEL DATE?
A passport is required for international travel, including for children and babies. Many countries require that passports be valid for six months AFTER THE RETURN DATE OF TRAVEL. So triple check those passport expiration dates now! And if you have additional questions, please contact me.
ARE YOU PREGNANT OR PLANNING TO BECOME PREGNANT?
Pregnancy Restrictions: Some trips have restrictions for pregnant travelers. For example, cruise lines will not board a passenger after her 24th week of pregnancy. Ask me for more details so we can sort out your travel plans!
Travel Insurance and Pregnancy: Please read your travel insurance’s coverage info carefully. In many policies, pregnancy is considered a pre-existing condition and is not a covered cancellation reason. In this case, you may want to consider "Cancel for Any Reason" coverage, an optional upgrade.
ARE YOU TRAVELING WITH A CHILD WITHOUT BOTH BIOLOGICAL PARENTS PRESENT?
Children under 18 years of age not traveling with both parents or legal guardian(s) may need to present a notarized letter from the absent parent/guardian granting permission to travel outside the United States. This letter should also authorize the adult to provide emergency medical care if necessary.
Please refer to this website for more information and sample letters.
HAVE YOU EVER BEEN CONVICTED OF A FELONY?
You may not clear customs if you have been convicted of a felony. Please contact your attorney to review the specifics of your case.
HAVE YOU EVER BEEN CONVICTED OF A DUI/DWI?
If you are traveling to Canada and have been convicted of a DUI or DWI you will likely not be admitted to the country.
A DUI/DWI is a felony in Canada and entrance to non-residents is tightly controlled. Please contact your attorney to learn more.
DO YOU OWE BACK CHILD SUPPORT?
You may be detained at customs if you owe child support. Please contact your attorney to discuss prior to payment.

"I don't need travel insurance. I am going on this trip no matter what!"
Famous last words.
But guess what? Travelers don’t get travel insurance because they think they’ll need it.
Like auto, health, and home insurance, travel insurance is for covering the unexpected.
Scenarios where travel insurance will cover your booty:
In other words … stuff happens. And when that “stuff” interrupts your travel plans—either before your trip takes place or even while you are traveling—travel insurance kicks into gear. It’s a small price to pay for peace of mind, for a major vacation investment.
Travel insurance is typically based upon your total trip cost, state of residence, and traveler age.
Select YES on the reservation form to receive a QUOTE for this important protection. We hope you don’t need to use it … but you will be so glad you have it in case you do.
• Two days before your vacation an immediate family member unexpectedly passes away
• Right before the trip you break a bone, end up in the hospital, etc.
• Your employer revokes your vacation time, or even worse you are laid off
• You’re enjoying your vacation when one evening you trip and fall and need medical attention
• Your flight is cancelled because the incoming plane is stuck in a city experiencing a snowstorm
The only thing worse than getting sick or injured on vacation is … having to pay the hefty bill that comes after. Most U.S. health insurance plans, Medicare, and credit card travel “coverage” DO NOT provide medical coverage outside the United States. If you're leaving the country without travel insurance then you're taking a HUGE risk. But travel insurance provides the health coverage you need, exactly when you need it most.

A passport is required for your travels. Do you have one yet? If you do, when does it expire?
A HUGE heads up: Current passport processing time is 6 to 8 weeks.
If you do not have a passport yet, this doesn’t allow much time for a hiccup. I strongly recommend that you get your passport applications submitted NOW! Make sure you get the passport BOOK, not the card—that won’t work for air travel.
If you do have a passport, check your expiration date—make sure that date is at least SIX MONTHS AFTER from the date you return home from your trip. If it isn’t, you likely need to renew now!
Do not wait, do not pass go, because there is NO get out of jail free card on this thing!
Other Need-to-Know Passport Notes:
1. Please verify that your passport complies with all local government requirements prior to departure. Click HERE to review passport + visa requirements for your destination.
2. Make sure that your passport does not have any water damage, tears, or missing pages. If it does, you may need a new one—so get on that, stat!
4. Do you have a passport book? A passport book, not a passport card, is required for international air travel. The passport card is only good for traveling by car or cruise to Mexico or Canada.
“Okay, okay, I’ll get my passport pronto … but, where do I apply?”
I recommend visiting your local post office to apply for your passport.
Check online for post offices near you that offer passport processing services. They will ensure that you have all of the proper documentation and application completed correctly. You can even have your photo done there, making it a one stop shop!
You can save time (and your sanity!) by making sure you have everything you need before your processing appointment by checking the U.S. State Department website.
Complete the registration form below to get started reserving your vacation.
There are 4 steps to the form. You must complete all 4 steps to complete processing.
Complete Step 1, click Step 2 and complete, and so on.
After you have completed all 4 sections, click submit.
After your form is received, I will process your reservation and send an invoice for you to approve.
Along with the invoice, you will receive the link to submit your deposit.
If you do not have a passport yet (or are traveling within the U.S.), you may leave that information blank.
Please note that a passport is required to travel outside of the U.S. by air.
